Root Cause Analysis Report
Identify incident causes, error categories, recurring patterns, and systemic improvement recommendations.
Capabilities of Root Cause Analysis Report
Cause categorization
Auto-categorizes every incident into root-cause taxonomy (misconfiguration, privilege escalation, process bypass, vendor integration, data quality, user error).
Recurrence trend detection
Statistical detection of same root cause appearing in multiple incidents within rolling 90-day window.
Pattern analysis
Cross-incident pattern detection — surfaces systemic issues no single post-mortem would identify.
Improvement recommendations
Each recurring pattern auto-generates a recommended treatment plan: control update, process fix, training need.
Audit-grade root cause documentation
Immutable RCA narrative per incident with auditor-ready chain of evidence.
Powered by live Oracle Fusion / EBS data
SyntraFlow reads Oracle audit logs, transactions, BPM workflows, and configuration metadata in real-time. The Root Cause Analysis Report report is fed by that live ERP signal — not by manual data entry or scheduled batch ETL.
Oracle-native
Pre-built understanding of Oracle Fusion / EBS audit-log structures and business objects.
Real-time refresh
Report values update within minutes of Oracle activity — quarterly reports, daily reports, real-time alerts all from the same source.
Drill-down evidence
Every report value traces back to source Oracle audit-log evidence — one-click forensic verification.
Both Cloud + On-prem
Works for Oracle Fusion Cloud + Oracle EBS R12.1 / R12.2 / 12cloud — single platform for mixed estate.
When teams reach for this report
Post-incident review
Replace 60-min meetings with auto-generated RCA drafts that the team only needs to validate.
Systemic-issue identification
COO uses recurring-pattern detection to prioritize platform improvements ahead of next quarterly cycle.
Compliance reporting
Regulators and auditors get the systematic RCA summary, not 47 individual incident write-ups.
Continuous improvement
Engineering uses cause-category trends to allocate platform-hardening budget effectively.

How does SyntraFlow determine the root cause?
▼
SyntraFlow uses an Oracle-trained classifier that examines each incident's audit-log fingerprint, configuration state at time of incident, related transaction context, and resolution actions taken. The classifier maps to a 6-category root-cause taxonomy (misconfiguration, privilege escalation, process bypass, vendor integration, data quality, user error). Confidence scores are shown — low-confidence classifications are flagged for human review.
What makes a pattern "recurring"?
▼
A recurring pattern is the same root cause appearing in 3+ incidents within a 90-day rolling window, OR appearing in incidents across 2+ business units / Oracle modules. Both thresholds are configurable per organization risk policy.
Can we feed this report to an external auditor?
▼
Yes. Each RCA is timestamped, attributable to its source incident, and auditor-grade traceable back to the underlying Oracle audit-log evidence. External auditors typically accept SyntraFlow RCAs as primary evidence for SOX ITGC walk-throughs.
How does this connect to Risk Treatment Plans?
▼
Each recurring pattern can be one-click promoted to a Risk Treatment Plan in the Risk Management module — same workflow, same governance, same audit trail. The platform ensures every systemic root cause has a corresponding remediation plan with an owner.
